What is a Request for Information Letter?

A request for information (RFI) letter is correspondence from your insurance carrier asking you to provide additional documents, information, or evidence to support your claim. This is a routine part of the claims process.

Insurers typically request information within 10-30 days of receiving your claim or at various stages during claim investigation. Failure to respond within the specified deadline may result in claim denial for non-cooperation.

Common Information Requests

Proof of Loss Documentation

Photos, receipts, estimates, invoices, or other evidence of damage or loss.

Medical Records (Health Claims)

Medical bills, treatment records, physician statements, or prescription documentation.

Police or Incident Reports

Official reports documenting theft, accident, or other insured events.

Repair Estimates or Invoices

Professional estimates or completed repair invoices for property damage.

Proof of Ownership or Value

Purchase receipts, appraisals, or other documentation proving ownership and value.

Signed Statements or Forms

Proof of loss forms, sworn statements, or other carrier-required documentation.

How This System Prepares RFI Responses

Procedural Compliance Response

System prepares a structured, procedural response with the following fixed sections:

  • Header – Your contact information, insurer address, claim number, policy number
  • Acknowledgment – Brief statement acknowledging the information request (2 lines max)
  • Document List – Numbered list of enclosed documents (5 items max, no descriptions beyond document name)
  • Compliance Statement – Brief statement that request is fulfilled (2 lines max)
  • Contact Information – Your phone and email (3 lines max)
  • Closing – Standard signature block

Total response length: Maximum 20 lines. No narrative. No explanations. No volunteered information.

What RFI Responses Will NOT Include

  • No narrative explanations – System does not provide stories or explanations of loss
  • No volunteered information – System lists only documents that were explicitly requested
  • No additional documents – System prohibits including documents that were not requested
  • No emotional appeals – System uses factual, neutral language only
  • No admissions or speculation – System does not make statements about fault or cause

Critical: Avoid Over-Disclosure

⚠️ Over-Disclosure Can Harm Your Claim

One of the most common mistakes policyholders make when responding to information requests is providing too much information. This can harm your claim in the following ways:

  • Inadvertent admissions – Narrative explanations may contain statements that harm your claim
  • Inconsistent statements – Additional information may contradict earlier statements
  • Unnecessary scrutiny – Volunteered information may trigger additional investigation
  • Policy violation claims – Extra details may reveal policy condition violations
  • Coverage disputes – Additional information may raise coverage questions

How This System Prevents Over-Disclosure

This system enforces strict limits to protect you from over-disclosure:

  • Maximum 20 lines total per response
  • Maximum 5 documents listed
  • No narrative sections permitted
  • No free-form text generation
  • Fixed playbook structure only

Request for Information Response Deadlines

Typical Deadlines

  • Standard RFI: 10-30 days from letter date
  • Urgent RFI: 5-10 days (rare, typically for time-sensitive issues)
  • Follow-up RFI: 10-15 days (if initial response incomplete)
  • State-specific: Some states mandate minimum response timeframes

⚠️ Consequences of Missing Deadline

Failure to respond within the specified deadline may result in claim denial for non-cooperation or policy breach. Always check your specific letter for the exact deadline.

When System Will Refuse to Prepare RFI Response

⛔ Hard Stop Conditions

Even if letter appears to be a simple information request, system will refuse output if it detects:

  • Fraud language – Any mention of fraud, misrepresentation, or false statements
  • EUO requests – Requests for Examination Under Oath (even if combined with document requests)
  • Recorded statement requests – Requests for recorded or sworn statements
  • Reservation of rights language – Any indication insurer is reserving rights
  • Attorney involvement – Any mention of attorneys or legal counsel
  • Bad faith indicators – Language suggesting bad faith investigation

If any of these conditions detected, system will refuse output and recommend attorney consultation.
